Instructions
Will your student be under 18 yo when new student orientation begins? If so, we need you all to complete this waiver.
As part of Western’s Orientation Program, students will be participating in a variety of activities, including a Headwaters Service Project and Wilderness Pursuits Trip, that will require a parent or legal guardian to sign the attached waiver form. Another event happening on Sunday, Aug. 24, is University Day at Crested Butte Mountain Resort (CBMR.) All Western students are invited to check out the area when it's not covered in snow! This is a great way to decompress, hike Mt. Crested Butte and see some spectacular views of the West Elk Mountain Range or ride the bike park. CBMR requires a waiver for this event too.
Without these signed waiver forms, students will not be able to fully participate in our new student orientation program events.

You have three options:
- Download/sign the waivers and upload it here, through Canvas. (This option is great because students will probably need to learn how to do this for their classes...)
- Download/sign and email the orientation waivers to orientation@western.edu
- Or, if students will be arriving to campus with a parent or guardian, the waivers will be at Orientation Check-In at the University Center.

CRESTED BUTTE MOUNTAIN RESORT WAIVER
PLEASE NOTE THAT STUDENTS NEED TO HAVE THE CBMR WAIVER WITH THEM TO ACCESS THEIR TICKET AT CBMR FOR UNIVERSITY DAY.
Parents or guardians should sign the waiver and have the student keep the waiver with them until the day of event. Students will then turn the waiver into the CBMR Ticket Office when getting their ticket for the event.
